PERSONAL COACHING — 5 SESSION, 3 MONTH PROGRAM: $1295

THINGS TO NOTE:

  • 5 SESSIONS THAT YOU WILL COMPLETE OVER A 3-MONTH PERIOD

  • NUTRITIONAL ASSESSMENT (a comprehensive health survey with 300+ questions).

  • FOOD MOOD JOURNAL ASSESSMENT.

  • EXTENSIVE INTAKE FORM.

  • THE FIRST COACHING SESSION IS A 90 MINUTE INITIAL CONSULTATION. 

  • THE SECOND COACHING SESSION IS 90 MINUTES.  

  • SUBSEQUENT TWO FOLLOW-UP SESSIONS ARE 1-HOUR. 

  • LAST SESSION IS A 30 MINUTE TOUCH BASE AND GO-FORWARD PLAN.

  • ALL PERSONAL COACHING SESSIONS ARE ONE-TO-ONE AND CAN BE IN PERSON OR VIRTUAL.

  • I AM DOING EXTENSIVE RESEARCH BEHIND THE SCENES TO PREPARE FOR OUR SESSIONS. I AM INVESTIGATING YOUR FAMILY HISTORY, SIGNS AND SYMPTOMS, HABITS, & HOW YOUR BODY WORKS WITH NUTRIENTS AND POSSIBLE PRESCRIPTIONS YOU ARE TAKING.   

  • THIS IS AN INVESTMENT IN YOUR HEALTHCARE.  YOUR BODY IS A SERIES OF CHEMICAL REACTIONS AND ONE THING COULD BE OVERLOOKED THAT COULD BE NEGATIVELY CASCADING THE REST OF YOUR GOOD HABITS.  TOGETHER, WE ARE GETTING TO THE ROOT CAUSE SO YOUR BODY CAN START TO CHANGE FOR THE BETTER.


LAYOUT OF SESSIONS: 

SESSION 1 // INITIAL CONSULTATION (1.5 HOURS)

This is where it all starts!

After you’ve decided to work with me, you will be sent a few fun forms. Yay! These include an Initial Interview Form, Food & Mood Journal, Signs and Symptoms Questionnaire, and Disclaimer Form that you will fill out and return to me (all done via email and sent back online).  Please fill the forms out with all of the details you feel are relevant, the more information the better and return 72 hours before our first call. Before our Initial Consultation together, I will be studying these documents and putting together a foundational road map for us to work together.

This Initial Consultation is 90 minutes where we discuss your vision of health, set goals, and lay the framework from which we’ll use for our entire coaching relationship. 

We’ll do a deep dive into your questionnaire, food log, dietary assessment, past medical history, education individualized to your focus area, review of any labs available (as needed) and wrap up with agreeing upon several foundational goals to start you down your path.

SESSION 2 // NUTRITION PATH AND DEEP DIVE INTO SUPPLEMENTS AND PRESCRIPTION INTERACTIONS (1.5 HOURS)

After you’ve taken action with your foundational goals for two weeks, you’re ready for your 2nd appointment, which is all about the nutrition path, signs & symptoms analysis, supplements & prescription deep dive, and more education about nutrition, how to nourish your body, and eat well for life.  We’ll discuss any roadblocks you observed in the first two weeks and develop a plan to navigate those roadblocks.

I’ll be reviewing the signs & symptoms you filled out in the Nutritional Assessment Questionnaire with you and advising you on what they mean.  I’ll equip you with tools on navigating these opportunities with supplements (optional) and food options.  

Keep in mind, sometimes supplementation is required to help the body function until it can do it on its own through proper nutrition. 

We will be deep diving into how the timing of how prescriptions & supplements are taken and how you eat and when, could be impacting your overall health.

We’ll cover how to assess hunger cues, mind-body connection, and how to perform daily check-in’s — whatever is best to meet your individual needs.

  • Grocery shopping lists as needed

  • Wellness vision and weekly goal setting

SESSION 3 AND BEYOND // FOLLOW UPS AND ACCOUNTABILITY

After you’ve completed your second appointment, we'll have follow up sessions to see how things are working, what challenges you might be facing, and how to progress forward. 

Your third session will be a deep dive into the changes coming out of the first and second appointments.  After your third session, I will have you redo your Nutritional Questionnaire (NAQ) and your Food & Mood Journal to see where there are any opportunities still lingering.  Again, the more details the better.

The fourth session will be reviewing your NAQ and your Food & Mood Journal based off the changes established from the beginning.

Your fifth and final session will be discussing your goals and It may include specific nutrition education topics you want to learn about, customized meal plans (i.e. updates to your nutrition plan), functional nutrition and health strategies, healthy cooking education, motivational techniques, and more.
